The episode was directed by David Innes Edwards and written by Rachel Paterson. The events of this episode take place on 26 January, on the same day as the previous episode.

Plot

After punching Ross to the ground, as the sound of an ambulance approaches Base's house, Iain repeatedly tells him to get up to no avail. When Jan and another paramedic enter the house, Iain informs them to his futile attempts to revive Base and Ross' current condition. He is shocked when Jan immediately recognises Ross as her son. In the ambulance, Jan tries to calm Ross down as he resists treatment. Iain tries to warn her about his recent activities but she tells him to shut up.

In the emergency department, Marty wishes Charlie a happy birthday. Nearby, Duffy doesn't think it's his birthday but Jade reaffirms that it is. Duffy, confused, questions how she could have forgotten her own husband's birthday and worries about getting him a present. Meanwhile, Ross is taken in resus. When Elle questions Ross' involvement with heroin addict Base, Jan asserts that her son is not a junkie.

Meanwhile, in cubicles, Louise supervises Jade as she treats Fiona who injured her arm in a fall. After a quick assessment, Jade decides to take Fiona for an X-ray but warns her that she may have to wait, prompting an exasperated rant. Back in resus, Ross wakes up to find Jan by his bedside. When he spots Iain outside, he asks her if Iain works at the hospital. Outside the ED, Ethan leaves Alicia a voice message asking her how her fellowship is going and asks her to call him back. He decides to go for a run.

Jan informs Ross that the police will need to talk to him about what happened at Base's house. He pretends to be unaware of Base's death. Ross then claims that Iain harassed Base for months and left them terrified. Jan believes his version of events and questions Iain about his involvement with Base. Iain barges into resus and tells Jan that Ross is a drug dealer who allowed Base to overdose on heroin. When Jan takes Ross' side, Iain grabs Ross' bag and demands Jan to check it for drugs as he is dragged out of the building by security. Jan promptly informs the police.

On his run, Ethan stops to catch his breath when he hears a woman calling for help in a field. He finds a woman called Anna who has fallen off her horse. Back at the hospital, as Iain leaves the hospital, detective constable Kate Wilkinson arrives to arrest Iain on suspicion of assault. Before being taken into custody, Iain berates Jan for believing Ross. Back in the field, as Ethan checks the sensation in Anna's legs, he learns that she had her right leg amputated ten weeks prior.

In cubicles, Jade prepares to escort Fiona to have an X-ray but she tries to leave her bed. As Jade tries to help her up, Fiona bites her arm and throws urine at her. Horrified, Jade quickly leaves. In the ambulance, Ethan is forced to remove Anna's prosthetic leg as she bleeds from her stump. At the police station, Iain is offered a chance to call his solicitor; he is adamant that he is innocent and declines. He is then led to his cell. Meanwhile, Anna is admitted to the ED.

In the staff room, Jan tells Charlie that Iain has admitted assaulting Ross to the police. Charlie suggests taking Ross' claims with a pinch of salt, citing his son Louis as an example. Jan disagrees; having witnessed Iain's irresponsible actions over the past few months, believing that Iain attacked Ross does not seem like a stretch to her. When she reaffirms that she does not believe that her son is a drug dealer, Charlie suggests checking his bag. In the hospital toilets, Louise finds Jade in tears trying to clean her wound.

Back at the police station, DC Wilkinson interviews Iain. Iain loudly asserts that Ross is a drug dealer and tells her to look him up; she informs that they found nothing on him. In the ED, Ethan prepares Anna for a CT scan. As he performs the scan, she regrets continuing horse-riding in her condition and being so reckless. Ethan reassures her. In resus, Ross agrees to speak to the police and Jan helps him on his way to the toilet. Meanwhile, Ethan tells Anna that he wants to run a few tests to check the sensation in her back.

In cubicles, Louise cleans Jade's wound and tells her that Fiona's records show no sign of hepatitis or HIV. Jade thinks that she might have chosen the wrong job but Louise reassures her. After testing the sensation in her legs, Ethan tells Anna that he will send her to have an MRI. Having experienced this treatment before and being emotionally drained by constant uncertainty, she is apprehensive about being treated. However, Ethan tells her that he is concerned that she has a spinal injury and promises to speak to her once he knows more.

Louise confronts Fiona and her husband Harry and tells them that the trust will press charges. Harry attempts to justify his wife's assault by claiming that she has been in constant pain for a decade. Louise points out that she refused to see a doctor and Fiona asserts that they cannot help her. After her husbands persuades her to seek treatment, she agrees to see a doctor.

Nearby, Jan decides to check Ross' bag but he returns from the toilet as she starts unzipping. Emptying his bag onto the bed, he tells her that he hated the way his father treated her and asserts that he is nothing like him. Jan comforts him as he seemingly breaks down in her arms. However, he immediately loses consciousness. At the police station, Iain tells DC Wilkinson what he knows about Ross, such as the drug den fire in October. When she asks him if there is anyone who could possibly back up his claims, Iain says that only Base could. The interview is terminated.

Ross is taken into resus and Elle treats his symptoms as signs of an overdose, despite Jan's requests for another CT scan. When he reacts to naloxone, Jan begins to realise the truth. Meanwhile, Ethan tells Anna that her spinal injury is serious but it can be fixed with surgery. He also informs her that her horse has been found. As her mother arrives, Anna thanks him.

As Jan stands outside resus, Charlie shows her Ross' X-ray which shows that he had swallowed his remaining heroin. When she speaks to her son, she is disgusted when he shows no remorse for what he did to Base and rebukes his remarks about her abandoning him and his father to start a new family with her new wife. She subsequently leaves him.

At the police station, DC Wilkinson informs Iain that he is being released on bail after learning that Ross' medical records substantiate his story. She also implores him to seek help. Back at the ED, Duffy asks Louise how Jade is coping after the assault. Louise tells her that she is determined to improve the safeguarding of her staff. Nearby, Ethan receives a call from Alicia but he declines and deletes her number. As he leaves, Duffy is surprised to find a birthday card and present she bought for Charlie.

DC Wilkinson takes Iain to a visibly shaken Jan. She says that she will never forgive Ross for what he did and offers to drive Iain home. Outside, Iain receives a notification on his phone to confirm his first counselling appointment. As he and Jan leave, he promises DC Wilkinson that she won't be seeing him here again.
