"Nice to see the old place hasn't changed. Still an island of calm in a sea of chaos."
Charlie is surprised to see Dylan again.
"Nothing like easing yourself in, is there?"
"Back by special request, for one night only."
"Am I given to understand, Mr Parker, that you're a violent man?"
"You don't know the half of it."
"Has it occurred to you that might be part of the reason why Steven's here? Just a thought."
Dylan examines Stevo's pupils.
Dylan learns from
Lily that Stevo may have a head injury.
"Well, it would’ve been nice if Lofty had shared this with the wider community."
Dylan explains Stevo's condition to Parko and Jed.
Dylan tries to resuscitate Stevo.
Dylan suggests performing a few more cycles of CPR and then reviewing Stevo's chances.
Dylan is exasperated at Stevo's deterioration and the head injury revelation.
The team concludes that Stevo's injury is non-survivable.
Dylan and Charlie try to calm Parky down.
"Look at what you're doing to him! This isn't a war."
Dylan sees Jed picking up a scalpel.
"Why do you think Stevo didn't tell us what was wrong?"
"I don't know, do I?"
"I think you do."
"Because he wouldn't dare, would he? "In front of the old man. Come on, it's time to stop."
Jed begins to change his mind.
Dylan watches as Jed finally confronts Parky.
"It's a shame Steven had to pay the price for today, isn't it?"
"I'm not sure about this. I think last week should've been a one-off."
"Dylan, you're only doing a bit of locuming. It's not like you're being sectioned."
"OK, OK, I'll do it for Dervla."
"If it hadn't been for her, I'd have stayed in Mogadishu."
"No, you wouldn't, you hated it there."
"At least in Mogadishu they shoot first, ask questions later, you're spared all the small-talk."
"Trust me, this place is in your blood."
"Does that make me some kind of masochist?"
Louise refuses to allow Dervla to stay near reception.
"Ready and raring to go?"
"Uh, ask me in an hour."
Dylan suggests that he meets his new colleagues as he works.
Dylan lectures a patient presenting with swimmer's itch.
"Good to have you back, Dylan."
"Is this all that's wrong with him?"
Dylan meets
Lofty for the first time.
"Anyway, apparently, you're in the lead."
"I can't say I'm surprised. The place is in complete disarray."
"You discharged my patient without my permission?"
"I didn't know I needed it."
"Since you asked so nicely,
Connie."
"
Mrs Beauchamp."
"'Atypical and differential diagnoses in abdominal pain.' Are you, are you stuck on your teenage girl?"
Dylan disagrees with
Ash of respecting the wishes of a
patient who is a Jehovah's Witness.
"Are you always this cut-and-dried?"
"Yeah, always."
"You think it's my fault she's ill?"
"Part of the reason, yes."
Dylan asks Rita to send a fresh urine sample to confirm a porphyria diagnosis.
"You think it's porphyria?"
"Well, it would have been nice if the test had confirmed it but, yes."
Ethan praises Dylan's judgement.
Dylan reads Dervla's report from doggy daycare.
Zoe jokingly gives Dylan a report on his conduct.
"Sorry, have you listened to anything I said?"
"No. Why would I?"
Dylan hears that Lily is on the road to a full recovery.
Charlie expresses his pride in the team coming together despite their differences to help Lily.
Dylan is bemused to hear someone crying in a cupboard.
"Is everything alright in there?"
Dylan realises she is Gem and asks if she wants to see Iain or a female practitioner.
Dylan notices Gem rubbing her eyes.
Dylan inspects Gem's eye.
"A little bit early in the day to be drinking, isn't it? I mean the spearmint doesn't quite camouflage it, though I appreciate the effort."
Dylan advises Gem to keep still.
"I'm a little bit concerned about damage to your cornea. You sure you haven't scratched it?"
"I'd know if I scratched it, wouldn't I?"
Dylan cannot treat Gem's eye if he does not know what caused its condition.
"OK, you leave me no choice, I'm afraid."
"No choice about what?"
"Well, you'll see, won't you?"
Dylan uses eyedrops on Gem.
Dylan sees a corneal abrasion and tells Gem that her litmus test indicates an alkaline burn.
Dylan realises that the circumstances surrounding Gem's eye injury sounds eeriely similar to what caused Lily's injuries.
"Gem, I need you to tell me the truth. One of our doctors was involved in a car accident today. Dr Chao? Lily? Because I thought the two of you were pretty close?"
"There's no easy way to say this, so here goes."
"I think Gem may have been responsible for knocking Lily over."
Iain insists that Dylan is wrong about Gem.
Dylan receives a call from
Hazel.
Dylan is happy to babysit
Rihanna and asks Hazel which night she has in mind.
"No, Hazel, don't put her on the phone. It's absolutely pointless talking to infants on the teleph- Hello, how are you?"
Dylan asks Rihanna about her day.
"You don't understand a word I'm saying."
Cal desperately tries to get Dylan's attention.
Dylan hears a noise and sees someone slumped.
"Can we get some help here, and we need a trolley!"
Dylan rushes to Cal's aid.
"As far as I know. Yeah, don't worry about Ethan, let's concentrate on you, shall we?"
"I'm not going to let you die, Cal. I'm not going to let you die."
The team treats Cal's injuries.
"He's lost a lot of blood. Dylan, how long was he out there for?"
"Too long."
Dylan prepares to perform a thoracotomy.
Dylan makes an incision in order to fit Cal with a chest drain.
Dylan begins to open up Cal's chest.
Dylan concentrates on the procedure.
Dylan massages Cal's heart.
"...the way the world turns, and my stomach churns. When it finally hits me out of the sky, I knew this day would come. I'm on the front line...
Dylan continues to work on an asystolic Cal.
"Dylan... Dylan, he's lost too much blood."
"No, there's... there's a.... Hang on..."
"Where's Cal?"
"He was attacked."
"Attacked? What do you mean?"
"Well, we don't really know. We did what we could, Ethan."
Dylan watches on as Ethan demands to see Cal.
Jan hands Cat Barling over to Dylan and the resus team.
"Cat is an ex-theatre nurse. Um... Maybe best that she doesn't hear all the details."
"Let's estimate body weight at... uh... sixty-two-"
"S... sixty-five..."
"Sixty-five? Sixty-five."
"Rash? Rash? Rash? Do you need some fresh air? Do you want to step outside? It's perfectly fine, it's perfectly understandable."
"Step outside, gather yourself, come back in ready to work, OK?"
"Quite a lot of fluid. We'll need to push it, but we're walking a knife edge, so let's proceed carefully, OK?"
Dylan fits Cat with a central line.
Dylan is torn over delaying the
rapid sequence induction to allow Cat the chance to talk to her husband
Jonathan.
Jonathan speaks to Cat before she is intubated.
Cat's throat is too swollen for Dylan to intubate her.
Dylan is forced to resort to a cricothyroidotomy.
Dylan makes an incision in Cat's neck.
"Damn, I've nicked a vessel."
Dylan and Rash complete the cricothyroidotomy.
The eschar tissue in Cat's chest is making it too difficult for her to breathe on her own.
Dylan weighs his options.
"Well, we're down to the final option, aren't we then?"
"Let's prep for that then."
Dylan calls Alicia in to assist with the escharotomy.
"Where the hell are burns and plastics?"
Dylan asks Alicia if she has performed an escharotomy before.
Dylan explains burn shock to Alicia.
Dylan begins the procedure with an incision.
Dylan accidentally nicks an artery.
Dylan wipes the blood off his face.
Dylan concentrates on the escharotomy.
Cat shows no signs of improvement, making additional incisions necessary.
Dylan instructs Charlie to highlight any changes in Cat's condition.
Dylan guides Alicia through the procedure.
Cat's heart rhythm returns to normal.
Charlie feels Cat's central pulse return.
Dylan informs the team that Cat is stable.
Dylan praises and thanks the team for their work in resus.
"Poor Cat Barling... She stands a chance though now."
"It was... humbling to have you alongside."
