Synopsis[]

Elliot is asked to attend a "stress test" to determine his fitness to practise — a request even Mark was unaware of. When a major incident is declared following a serious crash on the M4, Joseph is left in charge of Darwin, and Greg is sent down to AAU to assist in treating the crash casualties with Elliot. Elliot treats Bernard Robson — the driver of the lorry responsible for the crash — who insists that he swerved to avoid hitting a dog. Greg is sceptical of Elliot's competence and questions his demand for an eye test for Bernard. However, Elliot's test leads to Bernard being diagnosed with Charles Bonnet syndrome which gains Elliot Greg's respect. On Connie's desk, Greg finds a copy of Elliot's appointment letter along with another recommending making a consultant redundant, leaving him to believe that Connie is trying to engineer Elliot's downfall. After confronting Connie, Greg realises that the stress of her father William's condition is affecting her judgement and offers to speak to her on Elliot's behalf, but Elliot is not sure if he even wants to continue working at Holby City anymore.

It is the day of William Chase's laparotomy, and Hanssen relieves Connie of her clinical and managerial duties to be with her father on Keller where she struggles with her inability of help and navigate William's dementia. William's hernia becomes incarcerated, and he is rushed into theatre, but Hanssen and Sacha's investigation of William's large bowel reveals several pre-existing issues, and William leaves surgery with a stoma. After William's dementia triggers a particularly serious episode of delirium, Connie returns to Darwin to distract herself, but William soon goes walkabout and is found having removed his stoma. Connie is overwhelmed by everything — her father's condition, her fracturing relationship with Elliot, and her insistence on urgent discharges resulting in a patient collapsing in the car park.

Ollie starts his AAU rotation aiming to make a good impression on Michael. To his dismay, Ollie is put on relative liaison duty when the major incident is declared, but he eventually impresses Michael by taking the credit for one of Penny's suture jobs. Once he is left to his own devices in theatre, Ollie marks the wrong kidney for removal and is unable to recall anatomical facts. Ollie continues to embarrass himself when he tells a patient that her sister had taken an overdose of antidepressants before she died in the crash. Michael reprimands Ollie and threatens to have him removed from AAU if he continues to be incompetent and deceitful. Ollie is approached by Sami's mother who thanks him for taking care of her son before his death. Before Ollie can explain his role in Sami's decline in health, Elliot intervenes and tells Ollie that part of being a good doctor is knowing when to left things go. At the end of their shift, Ollie apologises to Penny, and Penny offers to help Ollie study.

On Keller, despite their frequent flirting, Donna denies being attracted to Kieran. Kieran procures a Christmas tree for the ward, but he and Sacha are forced to shorten the tip to fit it in the staff room. Later, Donna and Kieran kiss under the mistletoe, but it quickly becomes something more romantic.
